Discussed standards 320B, 430 , 210 and 120.

Comments:
An unannounced monitoring inspection was conducted on January 19, 2023. There were 2 staff and 8 children present ( 21 points). Children were observed dancing, tablet educational activiites and freely playing. Two children records and 1 caregiver record and 2 household members were reviewed on this date. Licensing Inspector reviewed supervision, activities, equipment, diapering ,nutrition, children and caregiver records, emergency evacuation and shelter-in-place drill documentation, emergency supplies and required postings.

Information gathered during the inspection determined area of non-compliance. Areas of
non-complaince is noted on the violation notice.

Violations:
Standard #: 8VAC20-800-790
Description: Based on observation and staff interview, the licensee did not have telephone numbers posted for 911 or local dial number for police, fire and emergency medical responder.

Evidence: Staff confirmed the 911 or local dial number for police , fire and emergency medical responder were not posted at the time of the inspeciton.

Plan of Correction: Staff #1 emailed Licensing inspector the emergency number document that will be posted in the home.

Standard #: 8VAC20-800-830-C
Description: Based on record review and staff interview, the licensee did not ensure documentaiton of shelter in place was maintained .

Evidence: Staff #1 confirmed she did not have documentation of shelter in place drills.

Plan of Correction: Staff #1 posted the drill log and it will include documentation for 2 shelter in place drills annually.

Standard #: 8VAC20-800-100-A
Description: Based on record reviw and staff interview, the licensee did not ensure a physical examination was obtain with in 30days after the first day of attendance.

Evidence : Child #2 ( start date 01/29/2022) and Child #3 ( 11/14/2022) start date did not have a physical with in 30 days after the first date. Staff #1 confirmed.

Plan of Correction: Staff #1 contacted the parent to provide a physical.

Standard #: 8VAC20-800-70-A
Description: Based on record review, the licensee did not ensure before the child'd first day of attendance , parents shall be provided in writing the provision of family day home's emergency prepardness and response plan.

Evidence: Child #1, Child #2 and Child #3 did not have in there record provision of family day home's emergency prepardness and response plan. Staff #1 confirmed.

Plan of Correction: Staff #1 will provide to parents the Provisions of the emergency prepardness and response plan and have them sign it immediatley.

Standard #: 8VAC20-800-330-A
Description: Based on observation , the licenee did not ensure a landline phone is operable during the family day homes operating hours.

Evidence:
Staff #1 confirmed the landline phone was plugged in but was not operable.

Plan of Correction: Staff #1 stated the router had to be reset. Phone is operable . Landline phone will be checked every other week. The family day home did have a operable cell phone.

Standard #: 8VAC20-800-420-A
Description: Based on observation and staff interview, the licensee did not ensure rooms used by children in care shall be lighted with natural or electric lighting for the children's safety and comfort.

Evidnece: Staff #1 confirmed the light was not working in the bathroom. During the inspection, Staff #1 replaced the light bulb and the light in the bathroom did not turn on.

Plan of Correction: Staff #1 had a person fix the light . The light switch had to be replaced. The bathroom light is operating.
