Comments:
An unannounced renewal inspection was conducted today between the hours of 10:00 am through 11:30 am. There were a total of seven children (18 points) observed with two caregivers. A total of four children's files and three staff/household member's files were reviewed. The children were engaged in the following: painting, toy play, handwashing, diapering, lunch and other organized activities. Areas of non-compliance are identified in the violation notice. Violations:
Standard #: 8VAC20-800-710
Description: Based on observation, record review, and interview, prescription medication was not maintained in the original labeled container with a prescription label.
Evidence:
1. Child #3 (Start date-10/3/2022) has albuterol and a dispenser chamber on site. The albuterol was not in the original labeled container; therefore, it did not have a prescription label identifying the medication for Child #3. There was documentation of authorization from the parent and physician in Child #3's record for the medication. Caregiver #2 confirmed that the medication did not have an original labeled container with a prescription label.

Plan of Correction: The provider will ask the parents for the original labeled container with the prescription label attached.

Standard #: 8VAC20-800-920
Description: Based on observation, the menu for the current one week period was not dated.
Evidence:
1. The current one week menu was not dated. Documentation of the days of the week (Monday through Friday) were displayed on the menu only.

Plan of Correction: The menus will be corrected by writing the current date.

Standard #: 8VAC20-800-960-E
Description: Based on observation, bottles in the refrigerator were not labeled with the child's full name and date.
Evidence:
1. Three bottles for Child #2(Start date-01/03/2023) did not contain the child's full name and date. Child #2's bottles were only labeled with their first name.

Plan of Correction: The parents will write the child's full name and date on bottles everyday.
