Technical Assistance:
Provided consultation on:
1) The most recent inspection notice shall be posted
2) All caregivers, including substitutes and assistants, shall have documentation that they received orientation training by the end of their first week of assuming job responsibilities
3) Documentation of orientation training shall be signed and dated and kept in their record

Comments:
An unannounced renewal inspection was conducted from 1:00 - 2:30pm with the provider and her assistants. There were 12 children in care, ranging from 13 months to four years of age, for a total of 29 points. Children were observed napping, waking, and playing with age-appropriate toys. Five child records, five caregiver/household member records, the emergency drill log, daily attendance, first aid kit, and menu were reviewed. The provider is certified in Medication Administration Training (MAT) as of 1/18/2023 and can administer medication to children in care. Standard #: 8VAC20-800-100-A
Description: Based on a review of five child records, the provider failed to obtain documentation that each child has received a physical examination prior to the child's first attendance or within one month after attendance. Evidence: the record for child #5 (date of first attendance: 10/18/21) did not have documentation of a physical examination.

Plan of Correction: Will obtain a copy of the physical from the parents.

Standard #: 8VAC20-800-180-A
Description: Based on review of five provider/household member records, not all household members had a repeat tuberculosis (TB) test/screening at least every two years from the date of the previous test/screening. Evidence: the record for household member #1 had a TB test/screening dated: 11/10/2019.

Plan of Correction: The household member will obtain an updated TB.

Standard #: 8VAC20-800-210-B
Description: Based on review of documentation, and an interview with the provider, not all caregivers obtained a minimum of 16 clock hours of training annually in areas relevant to their job responsibilities. Evidence: the record for Caregiver #2 only contained documentation of seven hours of annual training.

Plan of Correction: Going forward, will ensure that all caregivers have documentation of the required 16 hours of annual training.
