Comments:

An initial inspection was completed on Monday, January 30, 2023. There were no children present during this inspection at the family day home. The inspector reviewed compliance in the areas of administration, household members, caregiver training, physical equipment and environment, preventing spread of diseases, emergencies, and nutrition. The inspector review caregivers and household members background checks and initial application required attachments submitted for review for approval. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #:	8VAC20-800-770
Description:	Based on observation and interview, the licensee did not ensure to have a working battery operated radio. Evidence: During the inspection on January 30, 2023, there was no working battery operated observation. Interview revealed that a working battery operated radio was not available for review.
Plan of Correction:	The licensee will purchase new batteries for radio to ensure is it working properly.

Description:	Based on observation and interview, the licensee did not ensure to post telephone numbers. Evidence: During the inspection on January 30, 2023, there was no observation of posted telephone numbers found. Interview confirmed the telephone numbers were not available for review during the inspection
Plan of Correction:	The licensee will type and post all emergency telephone numbers at the family day home.
Standard #:	8VAC20-800-330-A
Description:	Based on observation and interview, the licensee did not ensure to obtain a landline telephone. Evidence: During the inspection on January 30, 2023, there was no observation of a landline phone. Interview confirmed a landline phone was not available for review during the inspection.
