Comments:

An unannounced monitoring inspection was initiated and concluded on 2/1/2023. The inspector was on site from approximately 1:15 pm-2:25 pm. There were 5 children in care, ranging in age from 8 months to 2 years, with 1 caregiver supervising. The inspector reviewed compliance in the areas of administration, personnel, household members, physical environment and equipment, care of children, emergencies, caregiver training, preventing the spread of disease and nutrition. A total of 5 child records and 2 caregiver records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Please complete the plan of correction and date to be corrected for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word corrected is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Description:	Based on a review of five (5) children's records and interview, the center did not ensure to obtain documentation that one (1) child had been adequately immunized according to the requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health regulations before the first date of attendance as required. Evidence: 1. The record of child #4 (date of attendance: 1/9/2023) did not contain an immunization record. 2. The provider acknowledged that they had not obtained an immunization record for child #4.

Description:	Based on a review of two (2) caregiver records and interview, the provider did not ensure that the caregivers annually obtained a minimum of sixteen clock hours of training in areas relevant to their job responsibilities. Evidence: 1. The records of caregiver #1 and caregiver #2 contained 4.5 hours each of training in 2022. 2. The provider acknowledged that the caregivers had not obtained the required 16 hours of training.
