Comments:
An unannounced renewal inspection was conducted today, February 1, 2023. The inspectors were on site from approximately 9:50 am until 11:10 am. There were four children present with one caregiver supervising for a value of eight points. Children were observed playing with the kitchen set and creating play meals for the inspectors. Children also listened to a story and did an art project. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of four child records, two caregiver records and one household member record were reviewed. The inspectors interacted with the provider and children in care.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. If you have any questions regarding this inspection, please contact the licensing inspector.
The provider has requested a modification to the capacity of the license from five children to ten children. A special use permit for ten children was approved by the City of Richmond.

Violations:
Standard #: 8VAC20-800-830-B
Description: Based on review of the emergency drill log and interview, the provider did not ensure shelter-in-place procedures were practiced a minimum of twice per year.

Evidence:

The 2022 emergency evacuation log had one shelter in place drill documented on 02/21/2022 when a minimum of two shelter in place drills is required per year. The provider stated she had only conducted the one shelter in place drill for the 2022 calendar year

Plan of Correction: Per the provider: I will use the department's drill form that has a place for the two shelter in place drills to be records to help as a reminder.

Standard #: 8VAC20-800-100-A
Description: Based on review of four children?s records, the provider did not obtain documentation of one child?s physical prior to the child?s attendance or within 30 days after the first day of attendance.

Evidence:

The record of Child #4, enrolled 09/13/2022, did not have documentation of a physical exceeding 30 days from the first day of attendance.

Plan of Correction: Per the provider: I called the parent and she will bring me a copy of the physical for the child's file.

Standard #: 8VAC20-800-90-A
Description: Based on review of four children?s records, the provided did not obtain documentation that a child was adequately immunized, before the child attended the family day home.

Evidence:

The record of child #4, enrolled 09/13/2022, did not contain documentation of immunizations exceeding the first day of attendance

Plan of Correction: Per the provider: I called the parent and she will bring me a copy of the immunization record for the child's file.

Standard #: 8VAC20-800-210-A
Description: Based on review of two caregiver records and interview, the provider did not ensure caregivers obtained 16 hours of training annually in areas relevant to their job responsibilities.

Evidence:

1. Caregiver #1 and caregiver #2, both had 14 hours of annual training between the period of 02/11/2021 through 02/10/2022 when 16 hours was required.
2. The provider stated that caregiver #1 and caregiver #2 had not completed all 16 hours of annual training.

Plan of Correction: Per the provider: We will complete the required 16 hours of training.

Standard #: 8VAC20-800-270-A
Description: Based on observation and interview, the provider did not ensure firearms of every type and purpose were stored unloaded in a locked container, compartment, or cabinet, and apart from ammunition.

Evidence:

1. An unloaded firearm was observed in an unlocked bin on the top shelf of a closet in a bedroom. The bedroom door was locked, however the firearm was not locked in an approved container.
2. The provider acknowledged the firearm was not locked.

Plan of Correction: Per the provider: We will store the firearm in the locked safe.

Standard #: 8VAC20-800-270-B
Description: Based on observation and interview, the provider did not ensure ammunition was stored in a locked container, compartment, or cabinet during the family day home's hours of operation.

Evidence:

1. A box of ammunition was observed being kept unlocked in a case on the top shelf of a closet in a bedroom. The bedroom door was locked however the ammunition was not locked.
2. The provider acknowledged the ammunition was not locked.

Plan of Correction: Per the provider: We will put a lock on the case that contains the ammunition.
