Comments:
An unannounced monitoring inspection was conducted today from 10:45am-12:00pm. There were 6children (15 points) supervised by 2 direct caregivers. The home, 2 staff records, 4 children?s records, 2 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed engaging freely and playing with toys. Areas of non-compliance are identified in the Violation Notice. Thank you for your cooperation during this inspection. Violations:
Standard #: 8VAC20-800-920
Description: Based on observation, the menu for the current one week period was not dated.
Evidence:
1. The menu posted for the week did not contain the date for the week.

Plan of Correction: The menu will be dated for the week.

Standard #: 8VAC20-800-60-B
Description: Based on record review, children's records did not contain required information.
Evidence:
1. Child #2's (Start date-01/30/23) record did not contain the policy number for the health insurance.

Plan of Correction: The provider will get the policy number from the parents.

Standard #: 8VAC20-800-240-A
Description: Based on observation, areas and furnishings of the family day home, inside and outside, were not maintained in a clean, safe and operable condition.
Evidence:
1. The wood on the left edge of crib contains four inches of splintering wood.
2. The wall located on the left side of the door near the table used by the children contains chipped and peeling paint.
3. The baseboards located near the table that the children use contains peeling paint.
4.. The door frames contains peeling paint.

Plan of Correction: We have the facility cleaned professionally every 10 days. Home maintenance for peeled paint will be done asap. Outside area will be prepared for children?s play soon. Regarding the splintering wood on the crib we installed the teething guard for the toddler who was chewing on the edge of the crib due to teething but he ripped the guard as well so we transferred him to a sleeping mat to keep him safe. We will soon install new teething guards and not use the crib until then.

Description: Based on observation, potentially poisonous substances were accessible to children.
Evidence:
1. A bottle of air deodorizer was located on the sink in the bathroom that children use.

Plan of Correction: The air deodorizer will be locked up in the cabinet.

Standard #: 8VAC20-800-320-B
Description: Based on observation, child resistant covers were not installed for unused electrical outlets.
Evidence:
1. Two outlet covers were missing from electrical outlets.

Plan of Correction: Outlet covers were placed on the electrical outlets at the time of the inspection.

Standard #: 8VAC20-800-510-A
Description: Based on observation and interview, children were not provided individual cots for resting or napping.
Evidence:
1. The cots were not labeled for individual use. Caregiver #1 confirmed that the cots were not assigned for individual use by labeling.

Plan of Correction: The provider will individually label the children's cots.
