Comments:
An unannounced monitoring inspection was conducted today between 1:00 and 2:00pm. There were 11 children (32 points) present with the Provider and 1 assistant within the supervision guidelines. The children were observed being held and fed bottles and taking a nap. Positive interactions were observed during this inspection. A complete inspection of the family day home, children and caregiver/household member records, evacuation drill log, menu, attendance and emergency materials were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice. Thank you for your cooperation. Violations:
Standard #: 22.1-289.036-A
Description: Based on a review of caregiver records, all caregivers did not complete the central registry
background check every five years after the initial background check.

Evidence:
1. Caregiver #1's most recent central registry on file was dated 8/4/2017 during the time of the inspection.

Plan of Correction: I received the background check back and need to send it out again because it was missing information. I will send it out this week.

Standard #: 8VAC20-800-100-A
Description: Based on a review of children's records, 1 out of 3 children did not have documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.

Evidence: Child #3 (doe: 1/3/2023) did not have documentation of a physical examination on file during the time of the inspection.

Plan of Correction: I will ask the parents to bring this in.

Standard #: 8VAC20-800-60-A
Description: Based on a review of children's records, 1 out of 3 children's records were not up-to-date.

Evidence:
1. Child #1 (doe: 3/16/2020) did not have an annual update of required emergency information. The last update of Child #1's emergency information was on 2/25/2020.

Plan of Correction: I will ask the parents to update the emergency information today.

Standard #: 8VAC20-800-90-A
Description: Based on a review of children's records, 1 out of 3 children did not have documentation that the child has been adequately immunized before attending the family day home.

Evidence:
1. Child #3 (doe: 1/3/2023) did not have documentation of immunizations on file during the time of the inspection.

Plan of Correction: I will ask the parents for that information today.
