Comments:
An unannounced renewal inspection was conducted onsite on January 24, 2023. There were three children in care, ages two moths through two and one half years old, with the provider. Children and household records were reviewed. Activity areas inside and outside the home were reviewed. Children were observed in provider and self-directed activities. Violations regarding required postings, a child's record update and the first aid kit were cited and appear on the violation notice. The violations were discussed with the provider during the exit interview at the conclusion of the inspection.

Violations:
Standard #: 8VAC20-800-760-A-1
Description: Based on observation the center failed to ensure that the first aid kit contains all required items.
Evidence:
1 - There was one triangular bandage in the first aid kit where more than one is required.
2 - The provider stated that she had only one triangular bandage.

Plan of Correction: The correct amount of triangle bandages have been added to the first aid kit. All required items are now inside the kit.

Standard #: 8VAC20-800-780-B
Description: Based on record review and interview the provider failed to obtain the parents signed acknowledgment of annual review of emergency contact information.
Evidence:
1 - The record for Child #1 does not contain documentation of annual review.

Standard #: 8VAC20-800-790
Description: Based on observation and interview the provider failed to post the telephone number of the emergency back up caregiver as required.
Evidence:
1 - The telephone number for the emergency back up caregiver is not posted in the family day home.
2 - The provider stated that the information was not posted.

Plan of Correction: The emergency backup caregiver's number is now posted in the daycare.

Description: Based on observation the provider failed to post the most recently issued license in the family day home.
Evidence:
The most current license was not visibly posted in the center.

Plan of Correction: The most recent license is now posted and visible within the facility.

Standard #: 8VAC20-820-120-E-2
Description: Based on observation the provider failed to ensure that the findings of the most recent inspection of the facility is posted in the family day home.
Evidence:
The findings of the most recent inspection are posted as required.

Plan of Correction: The findings of the most recent inspection information are now posted on my corkboard in the facility's entrance hallway.
