Technical Assistance:
1. Discussed court custody documents required if a parent does not have legal custody or there are visitation restrictions.
2. Reviewed requirement for posting the inspection report in the home, typically near the license.
3. The inspector will provide attendance template, and information for County zoning.

Comments:
An unannounced renewal inspection was conducted at 12:20pm. Observed 4 children = 8 points with the Provider. A household member was also present today. Ratios and supervision were in compliance. Children were napping during the inspection. Lunch served today included: macaroni + cheese, broccoli, apples, and milk. The provider participates in the USDA food program. The daycare areas of the home were found to be clean and sufficiently supplied with toys and equipment for the children. Annual training requirements were exceeded. Areas of non-compliance with standards and laws reviewed were found. Questions about this inspection may be directed to pamela.sneed@doe.virginia.gov

Violations:
Standard #: 8VAC20-800-780-B
Description: Based on records reviewed, it was determined that 2 of 5 children?s records did not include an annual review of emergency contacts signed by parents.

Evidence:

1. Child #1 ? Parents have not signed the file to indicate emergency contact information has been reviewed since the child enrolled 9/28/21.
2. Child #2 - Parents have not signed the file to indicate emergency contact information has been reviewed since the child enrolled 10/18/21.

Plan of Correction: The provider said she will have parents review + sign the record annually.

Standard #: 8VAC20-800-100-A
Description: Based on records reviewed, it was determined that 3 of 5 children?s records did not have documentation of a physical exam on-file within the 1st 30-days of attendance.

Evidence:

1. Child #1 ? No physical exam on-file. The child began attending on 9/28/21.
2. Child #2 ? No physical exam on-file. The child began attending on 10/18/21.
3. Child #3 ? No physical exam on-file. The child began attending on 8/23/22.

Plan of Correction: The provider said she has been requesting physical exams but the parents have not given them to her. She will follow-up parents to obtain the documents needed. In the future, the provider will get the physical exams within the 1st 30-days.

Standard #: 8VAC20-800-50-A
Description: Based on records reviewed, it was determined that a record of daily attendance has not been maintained. Evidence: Attendance records for the period of 9/15/22-2/5/23 were missing and the provider indicated that she was unsure of the whereabouts.

Plan of Correction: The provider said she will do better with keeping daily attendance records.

Standard #: 8VAC20-800-60-B
Description: Based on records reviewed, it was determined that 3 of 5 children?s records did not have documentation of required information.

Evidence:

1. Child #1 ? Parents have not signed the liability insurance form.
2. Child #4 ? 1 of 2 emergency contacts listed.
3. Child #5 ? 1 of 2 emergency contacts listed.

Plan of Correction: The provider said she will have parents provide required information and sign forms as needed.
