Comments:

A monitoring inspection was conducted on February 13, 2023. There were 6 children present, with 2 caregivers supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies and nutrition. The children were observed engaging in free play. A total of 3 child records and 2 caregiver records were reviewed.Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations:

Standard #:	8VAC20-800-960-E
Description:	Based on observation, bottles were not refrigerated and labeled with the child's full name and the date. Evidence Bottles were observed on a high chair, and on the window sill without a child's full name or date.
Plan of Correction:	I will make sure all bottles have the child's full name and date.
Standard #:	8VAC20-800-100-A
Description:	Based on review of children's records, the provider did not obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance. Evidence Child #1 and #2, first day of attendance 12/2/2021, do not have documentation of a physical examination in their record.
Plan of Correction:	I will contact the family and get the physicals for the children. I will make sure to get physicals within 30 days of their first day.
Standard #:	8VAC20-800-120-B
Description:	Based on revie of assistant's record, the record is not complete. Evidence The record for assistant #1, did not contain verification of age, job title, date of employment, name, address and phone number of an emergency contact and documentation of any health concerns.
Plan of Correction:	I will obtain the required information and put it in the record.
Standard #:	8VAC20-800-320-B
Description:	Based on observation, child-resistant protective covers were not installed on all unused electrical outlets accessible to children under five years of age. Evidences Electrical outlet in the kitchen area, by the diapering table and in the play room did not have child-resistant protective covers making them inaccessible to children under the age of five.
Plan of Correction:	Corrected during the inspection. I will check every morning to make sure all outlets are covered.
Standard #:	8VAC20-800-680-A
Description:	Based on observation, the provider did not wash their hands with liquid soap and warm running water after diapering a child. Evidence The provider was observed not washing their hands after diapering a child.
Plan of Correction:	I usually always wash my hands after a diaper change. I just washed them now.
Standard #:	8VAC20-800-680-B
Description:	Based on observation, caregiver #1 did not ensure that a child's hands were washed with liquid soap and warm water after diapering. Evidence Caregiver #1 was observed not ensuring that a child's hands were washed with liquid soap and warm water after diapering.
Plan of Correction:	I will make sure all children hands are washed after diapering and toileting.
