Comments:
A subsidy health and safety (SHSI) inspection was conducted on 2/2/23. The provider was present with 7 children, ages ranging from7 months to 4 years. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety
regulations. Only subsidy requirements that are not covered in licensing standard were reviewed during this supplemental inspection. The areas of non compliance is outlined on the violation notice.

Description: Based on a review of recordkeeping and interview, it was determined that the vendor did not maintain a current written list of all children's allergies.
Evidence:
There was no written list identifying child A as having an allergy to soy.

Description: Based on a review of recordkeeping and interview, it was determined that there was no written care plan from a physician for child A , who has a diagnosed allergy to soy.
