Comments:

An unannounced renewal inspection was conducted on 2/14/2023 from 10:00am-11:30am. There were 3 children (8 points) supervised by 2 direct caregivers. The home, 4 staff records, 2 children?s records, 3 household member?s records, evacuation drills, and policies were inspected. Children were observed engaging freely and eating lunch. Description:	Based on record review and interview, the family day home failed to obtain the results of a criminal history records check for a caregiver prior to 1st day of employment. Evidence: There was no documentation of the results of a criminal history records check for caregiver #4 prior to their 1st day of employment. Caregiver #4 employment date is 2/14/2023.

Evidence: Caregiver #4 record is missing documentation that two or more references as to character and reputation as well as competency were checked before employment. Description:	Based on record review, parents were not provided in writing of all required information before the child?s first day of attendance. Evidence: 1. The records for child #1 (enrollment date: 9/2022) and child #2 (enrollment date: 1/2023) did not contain a signed acknowledgment of having received the family day home administration of medications policy. 2. The record for child #2 did not contain a signed acknowledgment of the requirement to notify the parent when the child becomes ill and for the parent to arrange to have the child picked up as soon as possible if so requested by the home. 3. The record for child #2 did not contain a signed acknowledgment of the parent's responsibility to notify the family day home within 24 hours or the next business day if their child or any member of their immediate household contracted any reportable communicable disease.
