Comments:
An unannounced monitoring inspection was conducted today between the hours of 10:30 am through 11:30am. There were a total of nine children (17points) observed with one caregiver. A total of three children's files and four staff/household member's files were reviewed. The children were engaged in the following: toy play, singing, arts and crafts, hand washing, and other organized activities. Areas of non-compliance are identified in the violation notice. Violations:
Standard #: 8VAC20-770-40-C-1-c
Description: Based on record review of four household members' records and interview, the provider did not obtain a Central Registry report within 30 days of residency of the family day home.
Evidence:
1. Household Member #2's record did not contain documentation of a Central Registry results.
2. Caregiver #1 confirmed that a Central Registry Search was not conducted for Household Member #2 within 30 days of residency.

Plan of Correction: The provider will submit a Central Registry Search for the Household Member.

Standard #: 8VAC20-800-920
Description: Based on observation, the menu for the current one week was not dated.
Evidence:
1. The snack menu was posted on a dry erase whiteboard in the family day home, but was not dated for the week.

Plan of Correction: The provider will date the snack menu for the week.

Standard #: 8VAC20-800-570-A
Description: Based on observation, the provider did not ensure that a caregiver did not exceed 16 points.
Evidence:
1. When the inspector arrived at approximately 10:30am, the provider had nine children and 17 points. Caregiver #2 was present in the home in another room of the family day home, but not within sight and sound of the nine children. Caregiver #2 was within sight and sound of the children by 11:15am. The provider was over points for approximately 45 minutes.

Plan of Correction: The provider will ensure that the they will not exceed 16 points. If this does happen, another caregiver is needed within sight and sound.
