An unannounced complaint inspection was conducted on 2/6/2023 to investigate an allegation received by the department regarding Care of Children. Based on evidence gathered through observations, interviews and documentation review, the complaint was determined to be valid. A violation notice was issued to the facility. An exit interview was conducted on 2/15/2023.

Standard #: 8VAC20-800-640-6 Complaint related: Yes Description: Based on observations, interviews, and documentation review, it was determined that the provider used a forbidden action: punishment for a toileting accident. Evidence: on 1/3/2023, at approximately 4:45pm, child #1, age 3, was sent to use the bathroom. Between approximately 4:45 - 5:00pm, child #1 had a toileting accident. Provider #1 admitted that at approximately 5:00pm, they stated, "you just peed" and spanked child #1 approximately 3 times using an open, cupped hand on the child's bottom. A witness confirmed that this occurred and stated that child #1 whimpered and sniffled, but did not cry and did not have any visible marks. Plan of Correction: Not available online.
