Comments:
An unannounced monitoring inspection was conducted on February 14, 2023 from 10:40 a.m. to 11:30 a.m. There were six children present, ranging in ages from one to three years old with two staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and two staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-800-800-B
Description: Based on review of records and interview, the provider failed to review the emergency plan at least annually.
Evidence
1. The date of the last review of the emergency plan was 12/05/2021.
2. The provider confirmed that the plan had not been updated since 12/05/2021.

Description: Based on review of records and interview, the provide failed to obtain documentation of a physical examination prior to a child?s attendance or within 30 days after the first day of attendance.

2. Child #3?s start date was 01/09/2023. There was no documentation of a physical examination on file for Child #3.

Description: Based on review of records and interview, the provider failed to ensure each caregiver obtained a tuberculosis (TB) screening every two years.
