If provider is out and assistant is acting as sub, the assistant must have access to records and must be willing and able to communicate with the inspector as needed.
Emergency preparedness plan must be reviewed annually. Provider must document when the plan is reviewed.

Comments:
An unannounced renewal inspection was conducted on 02/13/2023 between the hours of approximately 10:55 a.m. and 11:55 a.m. There were 6 children present with the Provider and 1 assistant for a total of 12 points within the supervision guidelines. The children were observed preparing to go outside. A complete inspection of the physical plant, children and staff records, fire drill log, menu, attendance and emergency procedures were observed during this inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 22.1-289.036-A
Description: Based on record review, every applicant for licensure as a family day home and adult living in such family day home underwent background checks every five years.

Evidence: Household Member #1?s most recent central registry search was dated 06/01/2017.

Plan of Correction: I will contact them now and if they do not have it, I will send a new one today.

Standard #: 8VAC20-800-100-A
Description: Based on record review, the provider did not obtain documentation of physical examination for each child prior to the child?s attendance or within 30 days after the first day of attendance.

Evidence: Child #1 (start date:01/03/2023) did not have documentation of physical examination on file.

Plan of Correction: I will talk to her today and tomorrow I will get it.

Standard #: 8VAC20-800-90-A
Description: Based on record review, the provider did not obtain documentation that all children were adequately immunized, before a child may attend the family day home.

Evidence: Child #1 (start date:01/03/2023) did not have documentation of immunizations on file.

Plan of Correction: I will talk to her today and tomorrow I will get it.
