Comments:
The inspection was initiated on February 21, 2023 and concluded February 21, 2023. There were 3 children present with 2 staff supervising. The inspector reviewed compliance in the areas of administration, personnel, physical health of caregivers, caregiver training, physical equipment/ environment, care of children, preventing spread of disease, emergencies and nutrition. A total of 3 child records, 1 provider and 1 assistant record was reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program

Violations:
Standard #: 8VAC20-800-830-A
Description: Based on document review and staff interview the licensee did not ensure emergency evacuation procedures were practiced monthly with all caregivers and children in care during all shifts that children are in care.

Evidence: The licensee confirmed they did not conduct emergency evacuation drills during the months of May, June, July, and August 2022.

Plan of Correction: To make sure all the emergency evacuations are documented on the same paper and performed every month.

Standard #: 8VAC20-800-880
Description: Based on observation and staff interview the licensee did not ensure they were following the most recent, age-appropriate nutritional requirements of a recognized authority such as the Child and Adult Care Food Program of the United States Department of Agriculture (USDA).

Evidence: Three children were observed eating lunch and they were served crackers, cheese , ham, cookies and water. The licensee confirmed they only offered 3 of the five components required, by USDA, during lunch time.

Plan of Correction: The USDA lady from PCDC sent me paperwork to put on the fridge that shows all of the components of the food group.

Standard #: 8VAC20-800-320-G
Description: Based on document review and staff interview the licensee did not ensure fireplaces and associated chimneys shall be inspected annually by a knowledgeable inspector to verify that the devices are properly installed, maintained, and cleaned as needed.

Evidence: The licensee confirmed that the last inspection was conducted on January 26, 2022.

Plan of Correction: The chimney inspector did not realize if chimney was not being used it had to be still inspected every year. He will from now on.

Standard #: 8VAC20-800-500-C
Description: Based on observation and staff interview the licensee did not ensure stationary outdoor playground equipment shall be placed at least six feet from the perimeter of other play structures or obstacles.

Evidence: A composite piece / play structure was observed without a six foot perimeter from other play structures or obstacles.
1. There was approximately 16 inches between one side of the structure and another caterpillar type play structure.
2. There was approximately 2? 6?-5? 6? between the sides of the structure and the perimeter plastic boarder on all sides.

Plan of Correction: When being used we place the caterpillar on the ground. PCDC came and measured all the way around and will move mulch and move it out further.
