Comments:

A monitoring inspection was initiated on 02/23/2023 and concluded on 02/23/2023 from 10:30 AM until 11:45 PM. There were 11 children present, ranging in ages from 10 months to 3 years, with 2 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies and nutrition. A total of 5 child records and 2 staff records were reviewed. Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Description:	Based on record review and interview, the provider failed to obtain documentation before a child may attend the family day home that the child has been adequately immunized. Evidence: 1. The record for child 1 was reviewed on 02/23/2023. The first date of attendance was documented as 12/15/2022. The immunization record was dated 01/19/2023. 2. The record for child 2 was reviewed on 02/23/2023. The first date of attendance was documented as 03/14/2022. The immunization record was dated 04/01/2022.
