Evidence: 2 of 2 bottles in the refrigerator were not labeled with the full names and dates of child #4 and child #5.Plan of Correction: I will make sure bottles are label immediately. Immediately label.
Standard #: 8VAC20-800-100-A Description: Based on record review and interview, the provider failed to obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.
Evidence: Child #1's record did not contain documentation of physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance. Date of attendance for child #1 is 5/15/2019.Plan of Correction: I took it out folder to review forgot to put back.
Standard #: 8VAC20-800-60-B Description: Based on record review and interview, the parents were not provided in writing of all required information before the child?s first day of attendance.
Evidence: Child #5's record did not contain a signed acknowledgment for the authorization for emergency medical care should an emergency occur and the parent cannot be located immediately unless the parent presents a written objection to provision of medical treatment on religious or other grounds. Child #5's date of attendance is 9/12/22.Plan of Correction: Will inform parents first day of care. Parent sign and put in folder.
Standard #: 8VAC20-800-280 Description: Based on observation and inteview, potentially poisonous substances, materials and supplies such as, but not limited to, cleaning agents, disinfectants, deodorizers, plant care chemicals, pesticides, and petroleum distillates were not stored away from food in areas inaccessible to children.
Evidence: A bottle of hand sanitizer was next to three containers of infant formula and infant oatmeal in a white cabinent next to the refrigerator accessible to children.Plan of Correction: Immediately moved hand sanitizer.
