Comments:
The licensing inspector initiated a subsidy health and safety (SHSI) unannounced inspection on 01/05/2023 in response to a complaint received on 01/04/2023. A follow-up inspection was completed on 01/23/2023 and 02/14/2023. A licensing inspection was also conducted on this date. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection. Areas of non-compliance are identified on this Violation Notice.
Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 10 calendar days from today. Specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Standard #: 8VAC20-790-180-B
Description: Based on observation and record review; the provider failed to maintain a written hard copy record of daily attendance that documents the arrival and departure of each child in care as it occurs.

Evidence:

1. The attendance sheet in use by the provider on 01/05/2023 did not indicate arrival and departure of each child. The sheet had columns labeled ?Names of Children in Attendance?. There was a place for a handwritten date above each column and twelve rows below each column.
2. The column dated ?1/3/23? lists the names of twelve children and their ages underneath; however, there are no indications of departure.
3. The next column on the sheet is dated ?1/? and lists the names of twelve children and their ages underneath with no indication of departure.
4. Fifteen children were observed to be in care on 01/05/2023; however, there was no documentation on the attendance sheet for this date, nor any written record of the arrival of the children in care.

Plan of Correction: Provider has reviewed 8VAC20-790-180-B. Provider will ensure that it maintains a written hard copy record of daily attendance that documents the arrival and departure of each child in care as it occurs. Provider is in compliance with this standard at this time.

Description: Based on interview, the provider failed to ensure all staff obtained the department?s health and safety update course each year.

Evidence:

The provider and staff 2 did not complete the department?s health and safety update course for 2022.

Plan of Correction: Provider has reviewed 8VAC20-790-(2)-250-F. Provider has ensured that all staff have obtained the department's health and safety update course. Staff will begin reviewing trainings on the first of the year to ensure all mandated trainings are timely completed and not over looked. Provider is in compliance with this standard at this time.
