Comments:
The licensing inspectors initiated an unannounced renewal inspection on 2/23/2023 from 10:07am to 12:25pm. The inspection was completed on 2/24/2023. There were 10 children present with 2 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing, supervision, programming, medication, special care, emergencies, and nutrition. A total of 5 children?s records, 1 household member record and 3 caregiver records were reviewed.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law, and violations were documented on the violation notice issued to the program.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names, list staff by positions only.

Violations:
Standard #: 8VAC20-800-730-D
Description: Based on observation, the provider did not ensure medications were be kept in a locked place using a safe locking method that prevents access by children.
Evidence:
There were three children?s medications located in an unlocked backpack. The backpack was observed in the family day home?s backyard within the reach of children in care.

Plan of Correction: Lock backpack

Standard #: 8VAC20-800-90-A
Description: Based on record review, the provider did not ensure two of five children?s records contained documentation that the children have been adequately immunized according to the requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health regulations before attendance.
Evidence:
1. The record of child #1 (enrollment date: 10/24/2022) had documentation showing that the child has been adequately immunized, dated 1/12/2023.
2. The record of child #4 (enrollment date: 9/07/2021) had documentation showing that the child has been adequately immunized, dated 2/24/2023.

Plan of Correction: 1. Obtain documents right away.
2. Make sure those documents are on time.

Standard #: 8VAC20-800-170-A
Description: Based on record review, the provider did not ensure to obtain from each caregiver at the time hire a current Report of Tuberculosis Screening form published by the Virginia Department of Health or a form consistent with it documenting the absence of tuberculosis in a communicable form.
Evidence:
The record of caregiver #3 (employment date: 9/6/2022) had documentation of a tuberculosis screening dated 2/7/2023.

Plan of Correction: The original TB screening was done on time but had been misplaced. We will make sure to keep track of all paperwork.
