Comments:
A non-mandated monitoring inspection was conducted on February 27, 2023 from 10:00 A.M.-12:15 P.M. There were three children present, ranging in ages from seven months to three years of age, for a total of ten points, with one caregiver supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of three child records and one caregiver record were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-800-60-A
Description: Based on interview and review of children records, the provider failed to obtain all required information.

Evidence:
1. Child #1 and Child #2's file did not contain the name and policy number of the child's medical insurance.
2. Child #1 and Child #2's file did not contain address for one of the designated persons to contact in case of an emergency if the parent cannot be reached.
3. The provider verified that records did not contain the name and policy number for medical insurance, and an address for one of the emergency contacts.

Plan of Correction: The provider will contact the parents and request the information at pick up this evening.

Standard #: 8VAC20-800-70-A
Description: Based on interview and review of children records, the center failed to obtain all the required policies to the parents before the first day of attendance.

Evidence:
1. Child #1 and Child #2's first day of attendance was 02/20/2023. The file did not contain documentation of the providers policy for administration of medication.
2. The provider verified that the records did not contain the policy for administration of medication.

Plan of Correction: The parent will provide the parents with the policy for administration for medication and have place the signed copy in the children's files.
