Technical Assistance:
The next anticipated inspection will be the providers renewal inspection. Provider was reminded to have all renewal application documents submitted through the provider portal 45 days prior to expiration for processing. Links to the portal can be found on the VDOE website.

Comments:
An unannounced monitoring inspection was conducted today between 8:30am and 9:40am. There were 2 children present with the Provider for a total of 4 points within the supervision guidelines. The children were observed arriving at the family day home and practicing their numbers. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and staff records, fire drill log, menu, attendance and emergency procedures were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice.

Violations:
Standard #: 22.1-289.036-A
Description: Based on record review and interview, each adult household member residing in the family day home has not undergone background checks every five years.
Evidence: The providers fingerprint background checks were completed 2/20/2018. The provider confirmed that they have not renewed their fingerprint background check.

Plan of Correction: I will make an appointment to get it done this month.

Standard #: 8VAC20-800-830-C
Description: Based on record review and interview, documentation of shelter-in-place drills was not maintained.
Evidence: There is no documentation of a second shelter-in-place drill being practiced in 2022. The provider stated that a second drill was practiced, but they must have forgotten to document it.

Plan of Correction: I will make sure I write it down when we do it.

Standard #: 8VAC20-800-890
Description: Based on observation meals brought from home were not labeled with a name to identify the owner.
Evidence: Child #1's lunch box did not contain the child's name.

Plan of Correction: I always put a tag on the lunch box and the parents always take it off. Corrected during the inspection.

Standard #: 8VAC20-800-390-A
Description: Based on observation, children over the age of two do not have access to an individual location to keep clothing and personal belongings.
Evidence: The children's belongings (coats and lunchboxes) were observed in a pile in the corner of the providers playroom.

Plan of Correction: I had someone come and take measurements to build cubbies but they were too busy to build them for us. I will make a space somewhere for each child.

Standard #: 8VAC20-800-520-A
Description: Based on observation and interview, cribs were not provided for children birth through 12 months of age.
Evidence: Child #3 (11 months old) enrolled in the family day home when they were 10 months old. There is no crib available for the child to sleep in. The provider stated that the child sleeps on a mat with the rest of the children.

Plan of Correction: The child turns one this week.
