22.1 BACKGROUND CHECKS; CARBON MONOXIDE

Comments:
An unannounced renewal study was conducted from 10:30 am - 11:30 am. There were five children in care with the provider for a total of 16 points. The children were observed engaged in free play and resting. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records, one staff record, and one household member records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-800-750-A
Description: Based on observation it was determined that not all requirements regarding topical skin products were met. Evidence: A bottle of lotion, diaper ointment, medicated ointment, and saline spray was observed under a shelf of the changing table. None of these topical skin creams were accompanied with the appropriate written parent authorization.

Plan of Correction: The parents will provide written authorization for the topical skin products.

Standard #: 8VAC20-800-750-B
Description: Based on observation it was determined that not all topical skin products were kept inaccessible to children. A container of lotion, diaper ointment, medicated ointment, and saline spray was observed on a shelf under the changing table.

Plan of Correction: The topical skin products will be kept in a location inaccessible to children.

Standard #: 8VAC20-800-40-A
Description: Based on observation and interview with the provider it was determined that the provider failed to ensure that the total number of children receiving care did not exceed the maximum licensed capacity of the home. Evidence: The provider who is licensed to care for 4 children was observed caring for 5 children, ages 7 months, 7 months, 7 months, 3 years, and 4 years.

Plan of Correction: The provider will only care for the amount of children documented on their license.

Standard #: 8VAC20-800-60-B
Description: Based on review of five children's records it was determined that not all records were complete. Evidence: The records for child A (start date: 01/30/23), Child B (start date: 01/30/23), Child C (start date: 02/06/23), Child D (start date: 02/06/23), and Child E (start date: 02/22/23) were missing the following:
Emergency Contact Information, Parent's signed acknowledgement of the receipt of the written information for parents, written authorization for emergency medical care, written authorization if a caregiver is to administer medication, written authorization if the child is to participate in swimming or wading, written authorization if the child is taken off the premises of the family day home.

Plan of Correction: The provider will request that the parents of Child A, B, C, D, E complete the missing documentation for their file.

Standard #: 8VAC20-800-70-A
Description: Based on review of five children's records it was determined that not all records contained documentation that required information was provided to parent s before the child's first day of attendance. Evidence: The records for Child A (start date: 01/30/23), B (start date: 01/30/23), C (start date: 02/06/23), D (start date: 02/06/23), and E (start date: 02/22/23) did not contain documentation of the required operating information for parents.

Plan of Correction: The provider will provide the written information for parents to the parents of Child A, B, C, D, and E. The parents will sign the form and submit to the provider for their file.

Standard #: 8VAC20-800-280
Description: Based on observation it was determined that not all potentially poisonous substances were stored inaccessible to children. Evidence: 1. bottle of cleaner and a bottle of antiseptic were observed in an unlocked cabinet under the bathroom sink. 2. A can of air freshener and a can of disinfectant were observed on the counter of the bathroom sink.

Plan of Correction: The potentially poisonous substances will be kept in a locked cabinet.

Standard #: 8VAC20-800-540-B
Description: Based on observation it was determined that infant walkers were not prohibited in the family day home. Evidence: An infant walker was observed in the child care area of the family day home.

Plan of Correction: The wheels have been removed from the infant walker.
