Technical Assistance:
Discussed the online provider portal with the provider.
Discussed best practice regarding resilient surfacing and the trajectory of fall zones.

Comments:
An unannounced Renewal inspection was conducted this date. A total of 11 children (29pts) with the Provider and 1 Assistant was onsite today. The children were having lunch at the beginning of the inspection. Younger children were secured in high chairs and the older children were sitting calmly at the table. One infant was observed napping in a nearby crib and another infant was being held by one staff. Lunch today consisted of fish sticks, french fries, blueberries and strawberries with milk to drink. Cookies were offered after lunch. Ample amount of food to provide second helpings if requested. Walk through of all 3 levels of the home was conducted. All cribs were observed. Mulch was recently delivered for the outside play areas. After lunch, hand washing procedures were observed and the children transitioned to nearby toys to play with under the supervision of the assistant, while the provider guided the children in small groups to their nap areas. A total of 4 children's records, both staff and 1 household record was reviewed. The previous violations found during the November inspection were reviewed and corrective actions were verified. Standard #: 8VAC20-800-100-A
Description: Based on child record review, documentation of one physical examination was not obtained within 30 days of a child's attendance.
Evidence:
Child # 2 started care on 12/07/2022. Documentation of a physical examination has not bee obtained.

Plan of Correction: The parent will provide a completed health exam within one week.

Standard #: 8VAC20-800-90-A
Description: Based on child record review, documentation that a child has been adequately immunized was not obtained before one child began care.
Evidence:
Child #2 began care on 12/07/2022. Documentation of immunizations was not obtained until 12/19/2022.

Plan of Correction: I will require that parents provide a copy of immunizations before the child begins.

Standard #: 8VAC20-800-250-C
Description: Based on observation of the cribs, items were found across the top and hung from the sides of one crib.
Evidence:
Child #1 (9 months) was observed sleeping in a crib that had a mobile hanging form the top of the crib and a toy which was hanging on one of the sides of a crib.

Plan of Correction: I will remove these items from the crib today.

Standard #: 8VAC20-800-530-F
Description: Based on observation of cribs, a soft toy was found inside one crib.
Evidence:
A stuffed animal was found in a crib occupied by Child #1.

Plan of Correction: I will remove this toy today.
