Technical Assistance:
Discussed that reference checks are required for all assistants or substitute providers except for the provider's spouse, parent, sibling, or child.

Discussed background checks that will be expiring soon.

If parents are self employed, work from home, or are unemployed, this information should be indicated in the child's record to fulfill the standard requiring the place and and address of the parent's place of employment.

Comments:
An unannounced monitoring inspection was conducted today from approximately 11:00am-12:00pm. There were 7 children directly supervised by the substitute provider and subsequently the provider and substitute provider. The home, 2 staff records, 4 children?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications for children on site today. Children were observed playing freely and eating snack. There was an assortment of age appropriate materials for children. Areas of non-compliance are identified in the Violation Notice.

Violations:
Standard #: 8VAC20-800-730-D
Description: Based on observation, medication was not kept in a locked place using a safe locking method that prevents access by children.
Evidence:
There was an bottle of Acetaminophen was observed on a shelf in the downstairs pantry.

Plan of Correction: I put it far, I thought it was ok since the kitchen is locked.

Standard #: 8VAC20-800-100-A
Description: Based on record review, the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.
Evidence:
Child 4's record did not contain documentation of a physical examination by or under the direction of a physician.

Plan of Correction: I had the parent bring the paper yesterday.

Standard #: 8VAC20-800-120-C
Description: Based on record review and provider statement, there was no record to document the time of arrivals and departures of the substitute provider as required by 8VAC20-800-140 D .
Evidence:
At the beginning of the inspection the substitute provider was the sole provider of childcare on the premises. There was no documentation for this date to establish the amount of time the substitute provider was assuming the role of primary caregiver in the provider's absence. The provider stated that they do not use a form to document this occurance.

Plan of Correction: I printed the paper and will start doing that from today.

Standard #: 8VAC20-800-60-B
Description: Based on record review, children's records did not contain all required information.
Evidence:
Child 2's record did not contain the child's health insurance policy number.

Plan of Correction: The parent provided me with the policy number today.

Standard #: 8VAC20-800-70-A
Description: Based on record review, the provider did not inform parents, in writing, of all required information.
Evidence:
1. Child 1's record did not contain documentation that the parents have been informed of whether the provider carries liability insurance.
2. Child 2's record did not contain documentation that the parents have been informed of the emergency response plan of the family day home.

Plan of Correction: I spoke to the parent and they signed all the paperwork yesterday.

Standard #: 8VAC20-800-320-I
Description: Based on observation, the family day home did not have an operational smoke detector on the date of inspection.
Evidence:
The smoke detector in the family day home was beeping during the inspection, indicating that the batteries need replacing.

Plan of Correction: I changed the battery today.
