Comments:

An unannounced monitoring inspection was conducted today from 11:10am-12:15pm. There were 11 children (35 points) directly supervised by 3 staff. The home, 3 staff records, 5 children?s records, 2 household member?s records, evacuation drills, emergency supplies, and policies were inspected. here were no medications on site today. Children were observed playing freely and eating snack. There was an adequate supply of books, toys, and materials for the children. The home was clean and organized. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #:	22.1-289.035-B-4
Description:	Based on record review and interview, there was no documentation that a child abuse and neglect check was initiated for an employee that has resided outside the state of Virginia within the last 5 years. Evidence: Staff#2 (DOH: 01/04/2023) has resided outside the state of Virginia within the last 5 years, their records did not have documentation that out-of-state child abuse and neglect results were obtained.
Plan of Correction:	We will make sure Staff #2 completes it ASAP.
Standard #:	8VAC20-800-750-A
Description:	Based on observation, diaper ointments were not labeled with the child's name. Evidence: Seven containers of diaper ointment were not labeled with the child's name.
Plan of Correction:	We will make sure to label them.
Standard #:	8VAC20-800-760-A-1
Description:	Based on review of emergency supplies, the first aid kit was incomplete. Evidence: The first aid kit was missing adhesive tape.
Plan of Correction:	We will get a new supply ASAP.

Description:	Based on review of emergency drills, evacuation drills were incomplete. Evidence: An evacuation drill was not documented for the month of February 2023.
Plan of Correction:	We will make sure to complete them each month.

Description:	Based on record review, the family day home did not obtain documentation of additional immunizations once every six months for children under the age of two years. Evidence: Child #5's record contains immunization records that were last updated on 11/11/2021.
Plan of Correction:	We will make sure to remind parents to update their child's immunization record.

Description:	Based on observation, the diapering surface was not nonabsorbent. Evidence: The diaper changing mat had a tear across the length of the mat and therefore is no longer nonabsorbent.
