Comments:
An unannounced renewal inspection was conducted on 3/8/2023 from 10:30 am-11:50 am. There were 11 children (27 points) supervised by 2 direct caregivers. The home, 2 staff records, 5 children?s records, 2 household members? records, evacuation drills, emergency supplies, and policies were inspected. Children were observed engaging freely and completing worksheets. Areas of non-compliance are identified in the Violation Notice. Thank you for your cooperation during this inspection. Violations:
Standard #: 8VAC20-800-750-A
Description: Based on record review and interview, the provider did not obtain written parent authorization for topical skin products such as sunscreen, diaper ointment and lotion, oral teething medicine, and insect repellent.

Evidence: There was no documentation of written parent authorization for nine diaper ointments used for the children in care.

Plan of Correction: Will give the parents the form.

Standard #: 8VAC20-800-100-A
Description: Based on record review, the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.

Evidence: There was no documentation of a physical examination by or under the direction of a physician prior to child #1's attendance or within 30 days after the first day of attendance.

Plan of Correction: Remind parents.

Standard #: 8VAC20-800-320-H
Description: Based on observation and interview, all flammable and combustible materials such as, but not limited to, matches, lighters, lighter fluid, kerosene, turpentine, oil and grease products, aerosol cans, and alcohol were not stored in an area inaccessible to children.

Evidence: Fire matches were found in a drawer next to the kitchen sink.

Plan of Correction: Removed during the inspection.

Standard #: 8VAC20-800-690-E
Description: Based on observation of the diapering area and interview, the foot-operated receptacle used to dispose of soiled diapers was not hands-free.

Evidence: The foot petal on the foot-operated receptacle was broken, preventing the receptacle lid from opening on its own and requiring the caregiver to open the lid with their hands.

Plan of Correction: Replace this weekend.
