Technical Assistance:
Reviewed with provider the requirement that emergency radio should be battery operated in the event of a power outage.

Reminded provider that 2 shelter in place drills must be completed each year.

Comments:
An unannounced monitoring inspection was conducted with the provider on February 22, 2023 beginning at 11:59 a.m. and ending at 11:38 a.m. There were 9 children, ages 9 months, 20 months, 22 months, 23 months, 2, 2, 2, 3, and 3 years old in care of the provider and an assistant for a total of 23 points. The children were observed preparing for nap time. Three children?s records were reviewed. Two staff and one household member?s record was reviewed. There were no medications on site. The first aid kit and flashlight were reviewed and in compliance. The most recent emergency escape drill was documented on 01/11/23.

Standard #: 8VAC20-770-60-B
Description: Based on a review of documentation and interview with the provider, it was determined that the provider failed to obtain documentation of a sworn statement of affirmation for a staff prior to the first day of employment. Evidence: Caregiver A began employment in May 2022 and did not have documentation of a sworn statement of affirmation at the time of this inspection.

Plan of Correction: The provider stated that she would have the staff complete the sworn statement and a copy will be placed in the record.

Standard #: 8VAC20-800-90-A
Description: Based on a review of caregiver records and interview with provider it was determined that the provider did not obtain documentation of immunizations for Child A before the child attended the program. Evidence: Child A was enrolled 02/21/23 and the date of child file review was on 02/22/23.

Plan of Correction: The provider contacted the parent during the inspection and requested the missing documentation. Upon receipt the documentation will be placed in the child's record.

Standard #: 8VAC20-800-170-A
Description: Based on a review of documentation and interview with provider it was determined that the provider failed to obtain documentation of a Tuberculosis Screening at the time of hire. Evidence: Caregiver A?s record was missing the documentation of a Tuberculosis Screening prior to coming in contact with children. Caregiver A began employment on May 2022 and did not have documentation of a negative TB screening for this caregiver

Plan of Correction: The provider requested the staff have a tb screening conducted and the results would be placed in the staff record.

Standard #: 8VAC20-800-280
Description: Based on observation, the provider did not ensure hazardous substances were inaccessible to children. Evidence: The cabinet below the sink in the basement bathroom was not locked. The cabinet contained a bottle of Fabuloso multi-purpose cleaner and a can Microban bathroom cleaner.

Plan of Correction: The provider stated that she and the assistant would secure the cabinet before leaving the bathroom.
