Technical Assistance:
Clarification was offered on the following standards:
* 8VAC20-800-700-B. Long-term prescription and nonprescription drug use may be allowed with written authorization from the child's physician and parent.
* 8VAC20-800-700-C. When an authorization for medication expires, the parent shall be notified that the medication needs to be picked up within 14 days or the parent must renew the authorization. Medications that are not picked up by the parent within 14 days shall be taken to a pharmacy for proper disposal.

Comments:
A monitoring inspection was conducted on 03/08/2023 between the hours of approximately 10:15 am and 12:05 pm. There were 10 children present, ranging in ages from 2 months to 2 years (33 points), with 4 staff supervising. Children and staff were observed while playing with toys and completing art projects using tempera paint.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and 5 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations:
Standard #: 22.1-289.035-B-2
Description: Based on record review and interview, it was determined that the provider did not ensure that fingerprint-based criminal history check determination letter is obtained every five years for employees.
Evidence:
1) Fingerprint-based criminal history check determination letters were due for 3 caregivers: The most recent determination letter available in the record for Staff #1 was dated 02/07/2018; for Staff #2 was dated 02/16/2018; for Staff #3 was dated 03/05/2018.

Plan of Correction: Scheduled fingerprints.

Standard #: 8VAC20-800-180-A
Description: Based on record review, the provider did not ensure that caregivers obtain and submit the results of a follow-up tuberculosis (Tb) screening at least every two years.
Evidence:
1) A current Tb screening was due for Staff #2. The Tb screening form in the record for Staff #2 was dated 12/09/2020.

Plan of Correction: I will schedule an appointment.

Standard #: 8VAC20-800-320-G
Description: Based on documentation review, the provider did not ensure that wood burning fireplaces and associated chimneys were inspected annually by a knowledgeable inspector to verify that the devices are properly installed, maintained, and cleaned as needed.
Evidence:
1) The most recent fireplace inspection was dated 01/05/2022.

Plan of Correction: I will make an appointment.
