Comments:
A monitoring inspection was conducted on 03/09/2023 between the hours of approximately 1:25 pm and 2:10 pm. There were 4 children present, ranging in ages from 16 months to 3 years (9 points), with the provider supervising. Children and staff were observed while eating lunch, brushing their teeth and transitioning to naptime.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 1 child record and 2 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations:
Standard #: 8VAC20-800-780-B
Description: Based on record review, the provider did not review with the parent, annually, the emergency contact information to ensure the information is correct and obtain the parent's signed acknowledgment of the review.
Evidence:
1) The most recent review of the emergency information in the record for Child #1 was dated 06/21/2020.

Plan of Correction: The provider shall ensure that the emergency contact information is up to date, correct and signed by the parents.

Standard #: 8VAC20-800-830-A
Description: Based on documentation review, emergency evacuation procedures were not practiced monthly.
Evidence:
1) On the date of inspection, the 2023 emergency drills log provided for review, did not have an evacuation (fire) drill documented for the month of February 2023.

Plan of Correction: The provider shall provide and keep log for review of the monthly emergency evacuation practice, including fire dril.

Description: Based on record review, children's records did not contain all required information.
Evidence:
1) The record for Child #1 did not contain the first date of attendance.

Plan of Correction: The provider shall ensure the record for Child #1 will contain all required information, including first date of attendance.

Standard #: 8VAC20-820-120-E-2
Description: Based on review of required posted documents and interview with the program director, the provider did not ensure that all required documents related to the terms of the license were posted on the premises of the program.
Evidence:
1) On the date of inspection, the findings of the most recent inspection of the facility (08/17/2022) were not posted. 2) The inspection findings posted were dated 03/29/2022.

Plan of Correction: The provider shall ensure that all required documents related to the terms of the license are posted on the premises of the program.
