Technical Assistance:

2. Inspector will send a model form for emergency drills.
3. Discussed options for resolving the background check non-compliances.

Comments:
An announced initial inspection was conducted at 12:10pm. There were no children present during the inspection. All areas of the home were inspected. The daycare areas of the home were found to be clean and sufficiently supplied with toys and equipment for the children. All areas of the standards were reviewed with the provider. Areas of non-compliance with standards and laws reviewed were found. Questions about this inspection may be directed to pamela.sneed@doe.virginia.gov

Description: Based on interview conducted, it was determined that an adult listed as a household member on the initial application did not have a completed criminal record check. Evidence: Household member #2 did not have a completed criminal record check.

Plan of Correction: will send email regarding residential status for Household member #2

Standard #: 8VAC20-770-40-D-4-a
Description: Based on records reviewed, it was determined that an adult household member falsified their sworn disclosure statement. Evidence: Household member #6 signed a sworn disclosure statement on 12/6/22 indicating they have never been convicted of crimes in Virginia. The criminal record check completed 2/23/23 resulted in criminal convictions in 2012 and 2014.

Plan of Correction: new updated form will be signed and sent

Standard #: 8VAC20-800-760-A
Description: Based on observations made, it was determined that required first aid supplies were not in the home. Evidence: There was no first aid tape, and no triangular bandage.

Plan of Correction: new first aid tape and bandage has been bought

Standard #: 8VAC20-800-770
Description: Based on observations made, it was determined that required emergency supplies were not in the home. Evidence: There was not a portable battery-operated weather band radio. The radio in the home did not have weather band.

Description: Based on observations made, it was determined that a piece of equipment has been identified by the US Consumer Product Safety Commission as being hazardous. Evidence: The brown wood crib in the daycare was manufactured in 1983, and is no longer considered a safe sleep space.

Plan of Correction: Brown wood crib has been disposed of and was replaced by a new one.

Description: Based on interview conducted, it was determined that there was not a landline telephone in the home. Evidence: The provider said she does not have working landline phone service.

Description: Based on observations made, it was determined that glass doors did not have decorative objects place on them. Evidence: The sliding glass door located in the main daycare room had no decals or decorative objects on the glass.
