Technical Assistance:
Discussion was held on the topics of current enrollment, current staffing, and training.

Comments:
This monitoring inspection was initiated by licensing staff and concluded on 03/07/2023, between the hours of 2 pm to 3:30 pm. There were 3 children present (7 points), ranging from toddler-age to preschool-age, with 1 caregiver supervising. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; special care and emergencies; and nutrition. A total of 4 children records, 3 household records, and 1 staff file was reviewed.

Information gathered during the inspection determined that there were/was (4) non-compliances with the applicable standards. These violations were documented on the Violation Notice issued to the home.

Violations:
Standard #: 8VAC20-800-830-B
Description: Based on review, shelter-in-place procedures were not practiced a minimum of twice per year.

Evidence -
1. On the date of inspection (03/07/2023), the posted drill log did not have a recorded shelter-in-place drill recorded for 2022.
2. This finding was confirmed by the FDH Provider.

Plan of Correction: At least 2 shelter-in-place drills will be conducted in 2023.

Standard #: 8VAC20-800-100-A
Description: Based on review, 1 of 4 children files indicated that the Provider did not obtain documentation of a physical examination (by or under the direction of a physician) within 30 days after their start date.

Evidence -
1. On the date of inspection (03/07/2023), documentation of a physical examination was not on file for Child #4.
2. The start date for Child #4 was listed as 11/14/2019.

Plan of Correction: Documentation of a physical examination will be obtained from the child's parent.

Standard #: 8VAC20-800-60-B
Description: Based on review, 1 of 4 children files indicated that not all of the required information/documentation was contained within each record.

Evidence -
1. On the date of inspection (03/07/2023), the medical insurance number for the policy of Child #4 was not available for review.
2. While the policy name was listed, the policy number was not listed for Child #4.

Plan of Correction: The medical insurance policy number will be obtained for the child's parent.

Standard #: 8VAC20-800-90-C
Description: Based on review, 1 of 4 children files indicated that the FDH did not obtain update documentation (every 6 months) for a child under the age of two years.

Evidence -
1. On the date of inspection (03/07/2023), the immunization page on file for Child #3 was dated - 07/20/2022.
2. On the date of inspection, Child #3 was under two years of age.

Plan of Correction: An updated immunization page will be obtained from the child's parent.
