An unannounced monitoring inspection was conducted on-site March 7, 2023. The provider was available during the inspection. There were 7 children present, ranging in ages from 4 months to 4 years, with 2 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records, 1 household member record and 3 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Standard #: 8VAC20-800-180-A Description: Based on a review of caregiver records and interview on March 7, 2023, the provider did not obtain a current Report of Tuberculosis Screening form for each caregiver, every two years from the date of the first screening or more frequently as recommended by a physician or the local health department.
Evidence: 1. The record of caregiver #2 contained documentation of a tuberculosis screening dated 12/30/2020. 2. Caregiver #1 acknowledged the repeat tuberculosis screening was not complete.Plan of Correction: Not available online. Contact Inspector for more information.
Standard #: 8VAC20-800-210-A Description: Based on a review of annual training and interview on March 7, 2023, the provider failed to ensure caregivers obtain a minimum of sixteen clock hours of training annually in areas relevant to their job responsibilities.
