Technical Assistance:
Discussed the menu needs more detail to see all fruit and vegetable components clearly.

Comments:
An unannounced renewal inspection was conducted today from 9:50am-10:32am with the provider on site. There were 6 children with 1 caregiver. A sample of 2 children's files and 4 caregivers/household member's files were reviewed. The children were engaged in the following: toy play, listening to the provider read a book and playing with play dough. Interactions between the children and caregivers were positive. The site was clean, organized and contained an abundant supply of developmentally appropriate materials. The physical space, attendance records, first aid kit, drill record and the Emergency Preparedness Plan were reviewed. There were no medications on site. Description: Based on observation, topical skin products were not locked and were accessible to
children.
Evidence:
1. In the restroom that children use, there were two Aquaphor ointments in low drawers that the children could access.

Plan of Correction: I will make a place for storing them

Standard #: 8VAC20-800-100-A
Description: Based on review of children's records, the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.
Evidence:
1. Child #1 (start date 9/09/2022) and Child #2 (start date 9/09/2022) did not have documentation of a physical examination.

Plan of Correction: Parent of child #1 and #2 is going to get P/E on 3/17/2023.

Standard #: 8VAC20-800-70-B
Description: Based on review of children's records, the provider did not obtain the parent's written acknowledgement of the receipt of the policy for administering medication.
Evidence:
1. Child #1 (start date 9/09/2023) and Child #2 (start date 9/09/2023) did not have the parent's written acknowledgement of the receipt of the policy for administering medication.
The first page was in the file, but the 2nd page was missing with the parent's signature.

Plan of Correction: 2nd pages of child #1 and #2's policy for administery medication have been obtained.

Standard #: 8VAC20-800-90-A
Description: Based on review of children's records the provider did not obtain documentation that the child has been adequately immunized according to the requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health regulations before a child attended the family day home.
Evidence:
1. Child 1 (start date 9/09/2023) and Child 2 (start date 9/09/2023) did not have signed immunization records signed by a licensed physician, the physician's designee, or an official of a local health department. The provider had a copy of the immunization records, but it was not signed and not on the current form approved by the Virginia Department of Health or a physicians form.

Plan of Correction: Parent of child #1 & child #2 is going to get the proper immunization records on 3/17/2023.

Standard #: 8VAC20-800-140-B-3
Description: Based on review of training certificates, the provider does not have a current certification in cardiopulmonary resuscitation (CPR), as appropriate to the age of the children in care.
Evidence:
1. The provider's certification expired on 9/01/2022. The provider took the online course with American Red Cross on 3/06/2023, but has not completed the skills session.

Plan of Correction: I will schedule in person class to complete CPR certification.

Standard #: 8VAC20-800-140-B-4
Description: Based on review of training certificates, the provider does not have current certification in first aid training.
Evidence:
1. The provider's first aid certification expired on 9/01/2022. The provider took the online course with American Red Cross on 3/06/2023, but has not completed the skills session.

Plan of Correction: I will schedule in person class to complete first aid certification.
