A monitoring inspection was conducted on February 22, 2023 approximately 10:00 am to 11:00 am. There were a total of 9 children with 3 staff members present. Children were observed during engaged in group activities and indoor play time. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition.
A total of 2 child records and 2 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 22.1-289.035-B-4 Description: Based on staff records review, the applicant did not authorize the family day home, to obtain a copy of the results of a criminal history record information check, a sex offender registry check, and a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years.
