An unannounced monitoring inspection was conducted today between the hours of 9:20 am through 10:30am. There were a total of 10 children (19 points) observed with two caregivers. A total of four children's files and three staff/household member's files were reviewed. The children were engaged in the following: toy play, writing, circle time, and other organized activities. Standard #: 8VAC20-800-60-B Description: Based on record review, children's records did not contain required information.
Evidence:
1. Child #1's record (Start date-11/28/2022) did not contain Parent's signed acknowledgement of the receipt of the Liability Insurance Declaration. The form was in the child's record, but not signed.
2. Child #4's record (Start date-12/12/2022) did not contain documentation of the medical insurance policy number.Plan of Correction: The provider will ask the parents for this information regarding the policy number and get the Liability Insurance Declaration form signed by the parent.
