Comments:
An unannounced renewal inspection was conducted today between 11:35am-1:00pm. There were 11 children (31 points) present with the Provider and 2 assistant s within the supervision guidelines. The children were observed playing with toys, diapering, eating lunch, and preparing for naptime. Positive interactions were observed during this inspection. A complete inspection of the family day home, children and staff records, evacuation drill log, menu, attendance and emergency materials were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice. Thank you for your cooperation. Violations:
Standard #: 22.1-289.035-A
Description: Based on a review of household member records, all household members did not complete required background checks every five years.

Evidence:
1. Household member #1 had a sworn statement on file that was dated 9/21/2017 during the time of the inspection.

Plan of Correction: I will have the household member fill a new sworn statement.

Standard #: 8VAC20-800-780-A
Description: Based on a review of children's records, 2 out of 4 children's records reviewed did not have an annual update of emergency information.

Evidence:
1. Child #1 (doe: 4/30/2020) did not have documentation of an annual update of emergency information at the time of the inspection.
2. Child #3 (doe: 1/18/2022) did not have documentation of an annual update of emergency information at the time of the inspection.

Plan of Correction: I will get the parents to sign the annual update for the children.

Standard #: 8VAC20-800-120-B
Description: Based on a review of assistant's records, all assistant's records did not contain the required information.

Evidence:
1. Assistant #1 (doh: 11/1/2022) did not have documentation of 2 out of 2 required references of character and reputation.

Plan of Correction: I will get the references for my assistant.

Standard #: 8VAC20-800-180-A
Description: Based on a review of adult household member records, all adult household members did not obtain a report of tuberculosis screening every two years from the date of the first screening or more frequently.

Evidence:
1. Household member #1 had a tuberculosis screening dated 7/14/2014 during the time of the inspection.

Plan of Correction: I will have the household member get a new tb screening.
