Comments:
An unannounced renewal inspection was conducted today, March 15, 2023. The inspector was on site from approximately 11:00 am until 12:45 pm. There were three family day home children and three resident children in care with one caregiver supervising for a value of 12 points. Children were observed playing with the kitchen set, farm animals and barn, and dolls. Later children danced, washed their hands, and prepared for lunch. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of three child records, one caregiver records and two household member records were reviewed. The inspectors interacted with the provider and children in care.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. If you have any questions regarding this inspection, please contact the licensing inspector.

Violations:
Standard #: 8VAC20-800-770
Description: Based on observation and interview, the provider did not maintain a working battery-operated flashlight that shall be kept in a designated area and be available to caregivers at all times.

Evidence:

The provider stated she could not locate the battery- operated flashlight.

Plan of Correction: Per the provider: I will buy a flashlight tonight from the store.

Standard #: 8VAC20-800-100-A
Description: Based on review of children?s records, the provider did not obtain documentation of a child?s physical prior to the child?s attendance or within 30 days after the first day of attendance.

Evidence:

1. The record of Child #1, enrolled 09/26/2022, did not have documentation of a physical exceeding 30 days from the first day of attendance.
2. The record of Child #3, enrolled 03/30/2022, did not have documentation of a physical exceeding 30 days from the first day of attendance.

Plan of Correction: Per the provider: I called the parents while the inspector was here to ask that they provide a copy of the physical for the record.

Standard #: 8VAC20-800-90-A
Description: Based on review of children?s records, the provided did not obtain documentation that a child was adequately immunized, before the child attended the family day home.

Evidence:

1. The record of child #1, enrolled 09/26/2022, did not contain documentation of immunizations exceeding the first day of attendance.
2. The record of child #3, enrolled 03/30/2022, did not contain documentation of immunizations exceeding the first day of attendance.

Plan of Correction: Per the provider: I called the parents while the inspector was here to ask that they provide a copy of the immunizations for the record.
