Comments:
An unannounced renewal inspection was conducted on March 14, 2023, from 9:55 a.m. to 10:35 a.m. There was one child present that was two years old with one staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of two children?s records and one staff record was reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-800-800-B
Description: Based on review of records and interview, the provider failed to review the emergency plan at least annually.
Evidence:
1. Records indicated that provider review the emergency plan on 3/5/2021 and 3/14/2023.
2. The provider confirmed that the emergency plan had not been updated annually.

Plan of Correction: We have marked our calendars with reminders to review and update Emergency Plan every year. For 2024 the plan will be reviewed and updated before March 14, 2024.

Standard #: 8VAC20-800-90-C
Description: Based on review of records and interview, the provider failed to obtain documentation of additional immunization once every six months for children under the age of two.
Evidence:
1. Child #1?s date of enrollment was 9/23/2021 and birthdate is 3/5/2021. The immunizations on file were dated 9/12/2021, 3/11/2022 and 3/13/2023.
2. The provide confirmed that no documentation of additional immunizations was obtain between 3/11/2022 and 3/13/23.

Plan of Correction: I have updated the Policies for termination of care in the Information for Parents forms to include "Parents are required to submit health and immunization records every six months from birth to 2 years of age and every year after their annual checkup. Failure to do so is a violation on the part of caregiver and will not be acceptable.
We are also setting up reminders on our calendars to remind parents so that any future violations can be avoided.
