Comments:
An unannounced renewal inspection was conducted today from 9:55am-11:07am with the provider on site. There were 6 children with 2 caregivers. A sample of 2 children's files and 2 caregivers/household member's files were reviewed. The children were engaged in the following: toy play, watching tv, eating snack, washing hands and coloring. The site was clean. The physical space, attendance records, first aid kit, drill record and the Emergency Preparedness Plan were reviewed. There were no medications on site. Violations:
Standard #: 22.1-289.035-A
Description: Based on caregiver record review and interview, the provider did not obtain the
updated central registry findings every five years.
Evidence:
1. Caregiver #2 had a central registry dated 6/23/2017. The provider stated she sent for it, but it was returned for missing information. She stated she will send it again.

Plan of Correction: I already resend the application with the missing information. As soon as I get the email from central registry I'll send it to the inspector.

Standard #: 8VAC20-800-80-A
Description: Based on review of children's records, the provider did not obtain from the parent the name and location of previous day care programs and schools the child has attended within seven business days.
Evidence:
1. Child #2 (start date 3/14/2022) did not have record of the child's previous day care programs and schools.

Plan of Correction: The parent already fill it out N/A that the child didin't attend to another daycare.

Standard #: 8VAC20-800-90-A
Description: Based on review of children's records, the provider did not obtain that the child has been adequately immunized according to the requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health regulations.
Evidence:
1. Child #1 (start date 3/01/2023) had an immunization record in the file, but it was not signed or dated by a physician stating the child was adequately immunized.

Plan of Correction: I spoke to the mom. I gave her the school entrance Health Form. She said will have the formfill it out by the pediatrician and have it ready by Monday.

Standard #: 8VAC20-800-180-A
Description: Based on review of caregiver records, the provider did not obtain for each caregiver a current report of Tuberculosis Screening form every two years from the date of the first screening.
Evidence:
1. Caregiver #2 had a Tuberculosis report dated 3/18/2021.

Plan of Correction: Caregiver #2 has an appointment on Friday for the TB test.

Standard #: 8VAC20-800-210-A
Description: Based on interview, the caregiver did not obtain a minimum of sixteen clock house of training annually.
Evidence:
1. The caregiver stated that they had not completed 16 hours of training in 2022 for herself or caregiver #2.

Plan of Correction: I'm already register for take the trainings in order to complete the sixteen hours. Caregiver #2 will register to the trainings too.

Standard #: 8VAC20-800-280
Description: Based on observation, potentially poisonous substances, such as, but not limited to, cleaning agents and disinfectants, were not stored away in areas inaccessible to children.
Evidence:
1. Two bathroom cleaning products were under a sink in the kitchen and the safety lock was broke and the products were accessible to children.

Plan of Correction: I already replace the safety lock for a new one. No products are accessible to children.

Standard #: 8VAC20-800-690-E
Description: Based on observation, soiled disposable diapers and wipes were not disposed of in a leak-proof or plastic-lined storage system that is either foot operated or used in such a way that neither the caregiver's hand nor the soiled diaper or wipe touches the exterior surface of the storage system during disposal.
Evidence:
1. The provider had a trash can for diapers with a swinging top and was not foot operated.

Plan of Correction: I already purchase a new one that is foot operated.
