Comments:
The inspector and licensing administrator completed an announced mandated initial inspection from 12:30pm until 1:45. There were 6 children in care for a total of 14 points. The area of the home, both inside and outside was inspected. The provider has the needed equipment for the family day home such as emergency supplies, toys, and required postings. The required background checks were reviewed. The provider completed FDH Phase II training on 02/09/2023.

The requirement of MAT training was discussed with the provider. The food program information was shared with the provider.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-800-730-A
Description: Based on interview and observation, medications for children and household members were stored together.
Evidence:
The provider showed the inspector the cabinet in the kitchen where medications are stored and stated that children's medications and household medications were both stored there.

Plan of Correction: The provider will get a separate lock box for children's medications.

Standard #: 8VAC20-800-240-A
Description: Based on observation, the inside of the home failed to be maintained in a safe condition.
Evidence:
The basement door is accessible to children. The basement steps are open and steep and the handrails are loose. The doorknob has a child-resistant cap on it; however a school-age child could open the door and or it could be left unsecured giving access to younger children.

Plan of Correction: The provider will get a hook for the door and place it high up on the door.

Standard #: 8VAC20-800-260-E
Description: The provider failed to empty a container of water accessible to children.
Evidence:
There is a baby pool full of rainwater on the deck.

Plan of Correction: The provider will dump the water from the baby pool when not in use.

Standard #: 8VAC20-800-320-E
Description: Based on observation, the prodder failed to have a barrier or screen around a portable heater that is accessible to children.
Evidence:
There is a portable heater in the corner of the living room that does not have a screen or barrier. The children have access to this room when going to the bathroom and to the are where they rest.

Plan of Correction: The provider is getting a heat pump installed and the portable heaters will not longer be used.

Standard #: 8VAC20-800-350-E
Description: Based on measurement, the provider failed to maintain hot water tap below 120 degrees.
Evidence:
The water in the bathroom that the children use measured 126 degrees.

Plan of Correction: The provider will have her husband adjust the water heater.
