Comments:

An unannounced monitoring inspection was conducted on 03/20/2023 between the hours of approximately 10:15 a.m. and 11:00 a.m. There were 10 children present with the Provider and 2 assistants for a total of 20 points within the supervision guidelines. The children were observed playing with puzzles and preparing to go outside. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and staff records, fire drill log, attendance and emergency procedures was completed during this inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Description:	Based on record review, the provider did not document in writing each annual review and update to the emergency plan. Evidence: The most recent documented review of the emergency plan was dated 06/15/2021.

Description:	Based on record review, documentation of orientation was not signed and dated by the provider and assistant. Evidence: Caregiver #1 (start date: 11/21/2022) did not sign or date their documentation of orientation. It was also not signed or dated by the provider.

Description:	Based on observation, a high chair safety strap was not used and securely fastened when the high chair was occupied. Evidence: Child #1 was seated in a high chair working on a puzzle. The high chair safety strap was not used.
