Discussion of the current Emergency Preparedness and Response Plan (EP&RP). This form does not need to be placed into every child's file and be signed by each child's parent. Please post one copy and have only the Provider sign it after each annual review. The Licensing Inspector will send a blank copy of the EP&RP to the Family Day Home (FDH) since the one currently in use is part of the written contract and registration forms designed by the Provider.

A review of the submitted FDH renewal application was conducted with the Provider.

A reminder of the annual review of each child's emergency contact information (provided by the parents) due this month (March 2023) for all applicable children.

Comments:
This renewal inspection was initiated by licensing staff and concluded on 03/23/2023, between the hours of 10 am to 11:30 am. There were 9 children present (24 points), ranging from infant to preschool-age, with 2 caregivers supervising. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; special care and emergencies; and nutrition. A total of 9 children records, 3 staff records, and 5 household files were reviewed.

The information gathered during the inspection determined that there was not a violation to an applicable standard, code, and/or law. A Violation Notice was not issued. If you should have any questions, please call or send an email to charlie.perkins@doe.virginia.gov. Thank you.
