Comments:

A monitoring inspection was conducted 03/23/2023. There were a total of 3 children (7 points) observed with caregivers present. The children were engaged in the following: toy play, games, movie time, snack, bathroom breaks, hand washing, and other organized activities. The inspector reviewed compliance in the areas of administration, personnel and household members, physical environment, care of children, handwashing, diapering, medication, emergencies and nutrition. A total of 3 child records and 2 caregiver/household member records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #:	8VAC20-800-60-B
Description:	Based on a review of records, the provider did not ensure that three of the children's records contained all required information. Evidence: The record of Child #1, #2, #3 did not contain documentation of the annual review of the child?s emergency contact information required by 8VAC20-800-840. Child #1 had a annual review dated 11/15/2021, child #2 had an annual review dated 6/2/2021 and child #3 had an annual review dated 9/9/2021.

Standard #:	8VAC20-800-240-A
Description:	Based on observation, the provider did not ensure that areas and furnishings of the family day home, were maintained in a clean, safe, and operable condition. Evidence: The back playroom door would not open because so many toys were blocking the way.
Plan of Correction:	We will clean the playroom.
Standard #:	8VAC20-800-280
Description:	Based on observation, the provider did not ensure potentially poisonous substances, such as, cleaning agents, stored away from food in areas inaccessible to children. Evidence: Woolight advanced was stored in the kitchen in an unlocked lower cabinet under the sink.
Plan of Correction:	I will lock the cleaning supplies.

Description:	Based on observation, the provider did not ensure child-resistant protective covers were installed on all unused electrical outlets and surge protectors accessible to children under five years of age. Evidence: 4 electrical outlets by the cribs, 1 by the TV near the cribs, and 1 by the bathroom were uncovered.
Plan of Correction:	I usually have plugs in. The kids take them out. I will put the plugs back in.
