Comments:
An unannounced monitoring inspection was conducted on March 28, 2023. There were 4 children (10 points) present with one caregiver. The home, children's records, caregiver records, household member records, first aid kit, menu's, evacuation drills and attendance logs were reviewed. There were no medications on site. The children were observed watching a video and playing with play-doh. Areas of non-compliance were cited on the violation notice. Violations:
Standard #: 8VAC20-790-180-A
Description: Based on a review of household member records, the provider failed to obtain for each household member a current Report of Tuberculosis Screening fore in accordance with the requirements in 8VA20-800-170.

Evidence:
Household Member #1's TB screening expired 06/29/2022.
Household Member #2's TB screening expired 05/24/2020.
Household Member #3's record did not have documentation of a TB screening or test.

Plan of Correction: I will have household members obtain current TB test or screening and will put results in the files.

Standard #: 8VAC20-800-830-A
Description: Based on a review of evacuation logs, the provider failed to ensure that emergency evacuation procedures shall be practiced monthly with all caregivers and the children in care during all shifts that children are in care.

Evidence:
The last practice evacuation was 08/26/2022.

Plan of Correction: I will ensure to complete fire drills each month and document that for the log.

Standard #: 8VAC20-800-100-A
Description: Based on a review of children's records, the provider failed to obtain documentation of physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.

Evidence:
Child #1's record does not include documentation of a physical exam by the 30th day of attendance. Child's first day of attendance was 01/11/2023.

Plan of Correction: I will ask the parents for the child's physical and put it in the child's file.

Standard #: 8VAC20-800-60-B
Description: Based on a review of children's records, the provider failed to obtain all required documentation for each child enrolled.

Evidence:
Child #1's record did not contain documentation of the names of persons other than the custodial parents who are authorized to pick up the child.
Child #5's record did not contain documentation of the address of two designated person to contact in case of an emergency if the parent cannot be reached.

Plan of Correction: I will get the information from parents and put in the children's files.

Standard #: 8VAC20-800-280
Description: Based on observation, the provider failed to ensure that potentially poisonous substances such as, but limited to cleaning agents, disinfectants, deodorizers shall be sorted in areas inaccessible to children.

Evidence:
Lysol was stored by the entrance to the home on a cabinet that was accessible to children.

Plan of Correction: The Lysol was removed and placed in an inaccessible place during the inspection.
