Comments:
An unannounced monitoring inspection was conducted today at 10:15am. There were11 children directly supervised by 3 staff. The home, 3 staff records, 5 children?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed napping in cribs, playing with manipulatives with staff guidance and watching a children's program. There was an assortment of age appropriate materials for children. The home was clean and organized. Areas of non-compliance are identified in the Violation Notice.

Description: Based on record review, the provider did not review the emergency plan at least annually.
Evidence:
The last documented emergency plan review was in August 2019.

Evidence:
Adult 1 and Adult 2's records do not have a documented date of employment.

Evidence:
Child 4's record did not contain the child's health insurance policy number.

Description: Based on record review, parents were not provided all required information in writing.
Evidence:
Child 4's record did not have documentation that parents have been informed of all required information for the family day home. The forms were observed in the child's record, but were not signed by the parent.

Description: Based on record review, the provider did not obtain documentation that the child has been adequately immunized according to the requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health regulations prior to attendance.
