Comments:
A monitoring inspection was conducted on 03/29/2023 between the hours of approximately 4:00Pm and 5:20 pm. There were 13 children present, ranging in ages from 5 months to 4 years (34 points) , with 3 staff supervising. The provider was over the approved capacity in their license.
Children and staff were observed while planting seeds, playing on the outside area, washing hands, eating ice popsicles.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and 3 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations:
Standard #: 8VAC20-800-890
Description: Based on observation, food brought from the child?s home did not meet all requirements.
Evidence:
1) On the date of inspection, one bottle with milk was observed in the refrigerator. This bottle was not labeled with the child's name and the date. The provider stated that the bottle belonged to Child #3.

Plan of Correction: I explained that the name was removed during washing. Now the parents put labels on the bottles.

Standard #: 8VAC20-800-900-B
Description: Based on observation, the provider did not ensure that children were not allowed to eat or drink while walking.
Evidence:
1) On the date of inspection, children were eating ice pops. The inspector observed at least 2 different children walking while eating their treats.

Plan of Correction: They were looking for tissues because the ice pops were cold and after the assistant gave them tissue, they sat down.

Standard #: 8VAC20-800-40-A
Description: Based on observation, the provider did not ensure that the total number of children receiving care at any one time does not exceed the maximum licensed capacity of the home.
Evidence:
1) On the date of inspection, the inspector observed 13 children in care in the family day home. The fdh's approved capacity is 12 children at any one time.

Plan of Correction: It was for a few hours of family emergency.

Standard #: 8VAC20-800-210-A
Description: Based on record review, caregivers did not obtain a minimum of sixteen clock hours of training annually in areas relevant to their job responsibilities.
Evidence:
1) On the date of inspection, certificates available for review in the records documented the amount of training hours for the year 2022 as follows:
for Caregiver #3 - 9 hours, and for Caregiver #4 - 7.5 hours.

Plan of Correction: They will get more classes this year.

Standard #: 8VAC20-800-250-E
Description: Based on observation, the provider failed to ensure that balloons, and empty plastic bags large enough for a child's head to fit inside shall be inaccessible to children under five years of age.
Evidence:
1) Plastic bags were observed inside a drawer in the bathroom cabinet of the care area. The drawer was not locked.
2) A balloon was observed on the floor of the care area.

Plan of Correction: It was a mistake from cleaning ladies. I took it from the drawer. I will wan them next and check after them to make sure it does not happen again.

Standard #: 8VAC20-800-280
Description: Based on observation, the provider did not ensure that potentially poisonous substances such as deodorizers are stored areas inaccessible to children.
Evidence:
1) A bottle of Glade deodorizing spray was located inside the cabinet under the sink in the bathroom area. The cabinet was not locked.

Plan of Correction: It was an empty bottle that cleaning ladies put on the cabinet. I put it in the trash in front of the inspector.

Standard #: 8VAC20-800-320-B
Description: Based on observation, child-resistant protective covers were not installed on all unused electrical outlets accessible to children under five years of age.
Evidence:
1) One outlet cover in the kitchen area (dining area for the children in care) did not have a protective cover.

Plan of Correction: The outlet cover had a problem. It is fixed now.

Standard #: 8VAC20-820-120-E-2
Description: Based on review of required posted documents the provider did not ensure that all required documents related to the terms of the license were posted on the premises of the fdh.
Evidence:
1) On the date of inspection, the findings of the most recent inspection of the facility (09/21/22) were not posted.

Plan of Correction: Just removed them to clean the windows.
