Comments:
An unannounced monitoring inspection was conducted on April 3, 2023 between 9:45am and 11:40am. There were 11 children present with 3 caregivers supervising within the supervision guidelines. The children were observed playing with various age-appropriate toys, story time, and a group music activity. Positive interactions were observed during this inspection. The inspector reviewed compliance in the areas administration, physical plant, staffing and supervision, programing, fire drill log, menu, first aid supplies, attendance, child and caregiver records, and emergency procedures. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the programs. Violations:
Standard #: 8VAC20-800-80-A
Description: Based on record review and interview, the provider did not obtain verification of the identity and age of the child within seven business days of the child's first date of attendance.
Evidence:
1. Child #1 (start date 8/1/22) did not have documentation on file of verification of their identity and age.
2. Child #2 (Start date 4/11/22) did not have documentation on file of verification of their identity and age.
3. Child #5 (Start date 11/7/22) did not have documentation on file of verification of their identity and age.
4. Provider acknowledged verification of identity and age was not obtained from the parents.

Plan of Correction: I will tell the parents to bring me their child's birth certificate.

Standard #: 8VAC20-800-330-A
Description: Based on observation and interview with the provider, a landline telephone is not available during the family day home's hours of operation.
Evidence:
1. No landline is available in the family day home.
2. The Provider acknowledge there is no landline available because she had it disconnected.

Plan of Correction: I will contact the phone company to have the line reconnected.
